How Care Carta decides

Passes when last_updated_on (YYYY-MM-DD or M/D/YYYY) is within 12 months before the observation date; fails when older. Indeterminate when the date is missing (FILE_VALUE_MISSING), unreadable (FILE_VALUE_INVALID), or in the future (NEEDS_HUMAN_REVIEW). Judged from the header, so a file with malformed rows is still judged.

Scope: CMS validates the date's syntax; CareCarta compares the date with the audit's observation date.
